Well it may be that she felt pressure to look 'presentable', whereas at home, she felt comfortable without it. It's one thing if everyone is used to seeing a person without makeup, but when that person suddenly goes out without, sometimes people say things like 'oh, you look tired', or some other comment to that effect...Women are judged more on their appearance, even by total strangers. The lippy probably just helped her feel more confident, simple as that.
The problem with white coats, is that they rub against stuff, picking up germs as the go. Most of us in the more acute specialities took to wearing scrubs and changed them throughout the day.

I'm assuming you're female @herbaceous. I don't know why I thought you were a bloke. I often get the gender wrong on this site. It all adds to the mystery.
Not sure I still follow. My wife went out where she knows no one and felt more confident because she's wearing lippy? Isn't that just circular and back to why/how? Isn't the inference that she does feel more attractive then wearing lippy - but won't admit that even to herself? And she doesn't need to feel attractive at home? What in wearing lippy made her feel confident?
I find this stuff fascinating and amusing - I think 'man' has an inherent knack of deceiving themselves.
