Sorry to stop the humour but here's the tech bit...
Anaesthetic comes in cartridges in set amounts so economy not really an issue.
If there's a large amount of infection then anaesthetic won't work well, you'll need antibiotics first.
Yes, definately easier and quicker if patient is numbed up.
Any dentist should be able to give effective pain relief via anaesthetic, occasionally some areas can be tricky but not impossible.
There are rigies in every business.
