Bellis perennis are well known for fasciation. I have a pic somewhere and I have often found it on primroses - one had more than 40 petals on one flower!
After being directed to this thread, I thought I should post the picture of the holly we get, its goes white due to lack of sun and we regularly get this fasciation. Its stunning at Christmas.
