The compost looks a bit tired, but for this year I would leave it as it is, just water regularly and add a bit of tomato feed or similar once every week or two (the cheap brands will do fine).
If it's been kept outside it's whole life it's probably a hardy variety (maybe Dollar Princess) but leave all the old growth on over winter because it helps to protect the plant.
Next spring you can prune the plant (hard as you like, they flower on the new growth) and repot with fresh compost. You can carefully tease away some of the old compost and prune any long straggly roots a bit if you want it back in the same pot. It should then be a more bushy shape for next year.
