What can I do to save my plant?

Ive been trying to find what type of plant this is for a while now but I have no idea i have had it for 3 years and it's grown a lot it started off tiny. I have never had any problems with it, it has always looked healthy and I've always cleaned it's leaves and took care of it. Does anyone have any idea why it is dying or going this colour and what I can do to help it and also if anyone knows what type of plant this may be, I would be grateful thank you.

Could be an oriental Lilly based on the folliage
if it is a Lilly they prefer a well drained soil so i would replace the compost and mix in some grit and give it a liquid feed of tomato feed to give it a boost
Not a lily, this thing has a short trunk from which the 2 stems are growing. Unfortunately I know very few house plants so can't help with ID. I'm sure someone will be along soon with an ID for you. I would ask though, does this pot it is growing in have drainage holes? Perhaps it's waterlogged through poor drainage.
Looks like a dracaena of some sort.
