Some of my alliums haven't formed flowerheads. Is that common or have I just got dud bulbs?

Depends what they are Ryan. Many flower later in the year, but they also don't all produce flowering heads at the same time.  It's only early May. Bigger, more mature, bulbs will flower better than smaller, younger ones too.

Also - if the soil is very rich, they may grow foliage at the expense of flowers.

Having said that - yes, you may have some bulbs which won't flower if they've been poor quality or damaged.

OK thanks for that. I planted some last year and don't think they've come up again this year. Is that usual?

Some might get eaten - squirrels are fond of the little ones as they're not buried so deeply, and some may rot etc. Some might get crowded out by other planting and get less light than they really need. It's not uncommon image
