Grapevine Dying?

I have a grapevine - about 20 feet long and approximately 30 years old. Until about 3 years ago it has appeared healthy and produced a modest crop of grapes. It is now failing to shoot at the furthest extremity and is doing so by an increasing amount each year. The rest of the vine appears healthy with no sign of disease.
I'm worried that what is happening may be terminal with no hope of recovery.
Any thoughts?
