Should I pick out the longer leaves by hand or lob off the whole lot in one go once bushy and expect new growth?

You pick by hand as you need it. Pick off the top when it looks as though it's growing too much up to make it bushier and stop it growing flowers.
Pick the larger, outside leaves first, leaving the younger, inner leaves to develop.
