Conference pear disease

My conference pear tree has started to lose leaves - they turn brown and the branch dies back. On some branches there are leaves at the top but along the branch is a orangey red lumpy scar. What is the cause of this and is there a cure? Can it be passed to other fruit trees nearby (such as a plum tree)?
