A Clematis hedge

I have a clematis hedge that is possible 40 years old and underneath the greenery and flowers are hundreds of dead branches forever falling to the ground. I am afraid to cut it back as it is the privacy hedge between my neighbours. What can I do with it to keep it alive and keep it flowering each year. In winter it is just looks like lots of dead branches but it comes to life each year.

    Thank you but it is now in flower and I have not the heart to ruin it this year. I will wait till later on this year.

    Cut it back as soon as flowering is over to give it the most time to recover 

    Feed it then as well
