Diseased apple tree

I have a 35 year old Cox's apple tree and throughout this summer the bark has been falling off in pieces about six inches square. The tree still has plenty of leaves, cropped quite well and has put on growth as usual. Is it doomed, if so what is the cause and would it be ok to plant another apple tree in the same place?

I think bark falling off can sometimes just be lack of drainage - has the wet weather this year upset it, do you think? Or it can be a pest, the flatheaded apple tree borer or similar borer, see this link....

How much bark has fallen off? Is it on a branch that you can remove?
