Rose Blackspot

I have a lovely floribunda Rose Gracie on our patio in a pot. For 2 years it has been magnificent as it should be being named after out late pussy cat. I have kept it well but not over watered, pruned it regularly every year and fed it very regularly. Suddenly this year, although it produced a magnificent show of blooms in June, despite being regularly dead headed second flush flowers were few and far between. It looks pretty sorry for itself with massive blackspot on almost every leaf, if I remove every diseased leaf there will be nothing left so what are my options please.
1/Do I remove the rest of the diseased leaves ?.
2/ Do I give it a light Autumn prune.?
3/Does it need to be pruned and then repotted, in which case I am nervous about removing it from the pot , do I just use Peat Free Multi purpose, when should this be done.?
I would appreciate any help I can get I really do not want to lose this plant which has great sentimental attachments.
