Vine weevil. You can get nematodes, but you need to have the right temps/conditions for them to be successful. The grubs do more harm than the adults, as they're underground and eat through roots. That damage is often only noticed when plants keel over because they have no root system left.
Improving the general health of the shrub will help. That's a lot of damage, so it suggests your rhodo is already struggling a fair bit.

They're mostly visible at night, but you can also see them during the day. If you have other planting too, there's likely to be damage on that, depending on what it is. Heucheras are particularly susceptible.
