Help - troublesome pear tree

2 years ago I purchased three trees - all bare root. A Pear, an Apple and a Plum.
Apple and plum are going great-guns, but the pear has hardly grown and I suspect this is due to the problem it has now had for 2 years. Due to work, life and other bits getting in the way I hadn't dealt with it... but now it is looking quite sorry for itself yet again.
So, I have 2 questions...
1. How do I stop the tree getting this annual disease? And what is it? By August all the leaves will have gone black and fallen off. It is very destructive, whatever it is.



2. I thought I had purchased a dwarf tree, but I am not sure I have looking at the plant passport !- could anyone offer advice on likely height of this tree (as it is very close to the side of the house, but clearly still only about 80cam tall!) - would be good to know, if it ever does grow!
