Diseased apple tree.

I planted a “James Grieve” apple tree 2 years ago. The soil is heavy clay and organic matter was dug at the time. It bore a few poor fruits last year and has not thrived
due to disease. I have had to cut back diseased, dead branches and the trunk is badly affected. Is this scab? Is any point in trying to save the tree?
