Succulent conundrum

Hi all
I'm a little confused what to do with this succulent. As you can see from the picture, the leaves at the top are really good, thick and healthy. The ones in the middle look to be dying then at the bottom it looks like there's new shoots coming out which is making the whole thing lean!!
Any thoughts on how best to deal with this would be appreciated!
