Dracena leaf drop - is it a pest?

I’ve got an old large dragon tree that has been dropping a lot of leaves lately. I’m sure it’s not over watering and I’ve now noticed that some of the leaves have yellow splodges on, can anyone identify if this is a pest or disease?
