Boring Insects?

Hi everyone, can anyone confirm what tree this is and what may have caused the holes?

Also, it looks like the leaves have a fungal infection too?

    The leaf damage could be physical, could be wind damage. Trunk looks dead or dieing.
    You could cut back to a stump. It may regrow.
  • If the trunk
    is that full of interesting boring beetle larvae then it’s on its way out ... it will already be ‘bored’ full of holes and beginning to rot. 

    Woodpeckers find the beetle larvae by tapping on the tree trunks and drilling a hole where it sounds hollow. They won’t have drilled holes into healthy solid wood.
