Plum tree help

Hi all, We inherited this Plum tree, we usually miss any plumbs because we've never really spent much time in the garden.
We had her pruned about 2 years ago. She does grow year on year and always has loads of fruit.
I know that the fruit needs thinning out.
Question is how do I thin it out? If I've got 4 growing on one central point (and they're roughly the same size) do I remove two or three? and if I do this will it help the current problems (some going moldy and what looks like the plums leaking) or is that a separate issue?
