I buy a lined A5 cheapo notebook each year and write in it each evening, what the weather's like, what I've done in the garden (or not done), what I've bought for the garden, where from and the price, what flowers have bloomed etc. I've got 10 years worth of records now and sometimes in the winter, I'll read back. I also take photos regularly, especially of new projects.
I use notes on my IPad but to be honest the list can get endless. Basically, I now just note anything I’ve planted in my garden to just keep a date of when I planted it so I can get a good idea on the plants performance over the years. This also helps with when to propagating plants as well
I wish I had the patience and diligence to keep notes. I seem to lack that discipline though! I bought a very groovy, hardbacked garden notes book years back, but it didn't last. Instead, I've managed to accumulate endless photos of plants and gardens I love, on my iPad, as well as an alarmingly long wish list of plants I need in my life.

I have a five year garden diary which I fill in if I've been in the garden.
I have a spreadsheet that lists what plants I have bought, from where and how much they cost.
I have a spreadsheet that lists where all my plants are in the garden, when they were planted etc. This also has a page that logs all the seeds I've sown, when they germinated and how many.
I have a sketchbook that has a plan of each of the borders.
I take photos through out the year which I save in virtual albums under the season.
Oh and a notebook that lists what jobs I need to do each month.
Thanks @dappledshade. I find it rather relaxing just to sit quietly in the evening and reflect on the day. The very fact of writing in a book (as opposed to typing on a computer/phone/laptop) calms the mind and the diary acts as a good reminder on what you have achieved (or not!) that day.
I have a daily diary on the computer and 3 spreadsheets - 1 for recording details of seeds sown (sowing date, germination, planting out, cropping etc.), 1 for monthly tasks, 1 a planting record for each area of my garden. I think I'm a bit ocd about this!
