Help with identifying and problem solving!

Please can someone help identify this plant and what's causing the leaves to disintegrate after small holes appear?
There are no signs of mites, flies or anything in and around the plant or under it's leaves. It usually flowers light blue clusters but hasn't this year...

    Possibly a leaf miner on the leaves?

    Is the pot that your plant is in ceramic or plastic gecolclough? Whichever material it is made from it is dark and will therefore heat up very quickly in this Summer weather we are experiencing and this can, effectively, 'cook' the roots of any plant in a similar container. A plain terracotta plant pot would perhaps be better, alternatively shade the dark container from direct sunlight.
