Dying Yesterday Today and Tomorrow

As you can see from the attached images - this plant has steadily been getting worse over the last 2 years or so. It is getting sufficient water, and I gave the plant some fertiliser twice in the last few months. The leaves have died back and it looks like the stems are gradually dying on one side of the plant. Any ideas on what is causing this? Any help would be appreciated.

Thanks, yes I noticed most posts were UK based after posting mine.

I am based in Cape Town. I will probably get in touch with a local gardening centre for help.
