Potatoes in Raised Beds

February's GW magazine had an excellent article by Kay Mcguire on comparing different ways of growing potatoes. One of the most successful was using a raised bed, where Kay planted 6 seed potatoes in a single one metre row, with no earthing up. This has raised the following questions:

1. At what depth should the tubers be planted? My raised beds are 12" deep

2. My beds are 40" square. The extrapolation of Kay's method is a grid of 36 tubers, which seems an awful lot for that area. Interestingly, Monty has a video wherein he plants only 4 tubers in a row. Assuming I am talking about first or second earlies, do people think a full bed of tubers is sensible, and at what density?
