Yes to all the above. This rose definitely needs a good feed in the spring as you can see from the yellow of the young leaves at the top. In general Hybrid Teas like this are not the longest-lived plants, but if this plant still has enough vigour to do a good show, then some hard pruning would be advisable to encourage the plant to develop young growth from low down. But do this judiciously to keep it going. Total renewal will probably take about three years.
