Diagnosing Apple Tree

Hello,

I'm wanting to find out whats wrong with an apple tree please?

A while back there was some black fungus growing out of the ground beneath it, it's not there anymore and may or may not have anything to do with the health of the tree?

About a year ago a branch died so it was cut off. Now two substantial limbs have died off. The rest of the crown still looks full and healthy, did it just not need those branches or is something up?

The fruit is still as good and plentiful as it's always been. The leaves have some spots on them, is this common, what is it, is there anything I can do?

    Sadly I don't know, as It's not actually my tree, I hadn't seen the tree when the fungus was beneath it. Sadly all I know is, it was black! SorryΒ image
