I can't help with the cause, but I had almost exactly the sam problem with 'Bloodgood'. It had been fine for several years, but this year only one small branch had any leaves on it. All the rest had died. Maybe the winter weather conditions did for it.
I have also lost a few branches on one of my Acers. No real idea why other than the possibility it became far too dry during the long dry spell we just had.
